Adult Health and Chronic Disease
Blood pressure, blood sugar, and A1C screening- both in office and mobile clinics (call our office for more information about dates and locations).
Fit Kit testing-an at home test to detect for blood in the stool with results and recommendations for appropriate follow up when needed.
Healthy U Classes - a 6 week, in person, health promotion class designed to encourage adults to take a more proactive role in their own health management. (Call our office for dates and locations).
Functional assessment for Medicaid eligibility for Nursing Home care or Home and Community Based Waiver Program.
Ryan White Case Management 

General
CPR (HeartSaver and BLS) both in person classes or skills pass off through the American Heart Association.
